The Central Highlands, Victoria, General Practice Grant Program (The Grant Program) has been awarded annually since 2014, and was established through the transfer of retained equity from the Central Highlands General Practice Network (CHGPN) to Health Education Australia Limited (HEAL). The aim of the Grant Program is to support general practices in the historical CHGPN catchment to improve chronic disease management through systems development, capacity building, whole of practice learning and knowledge transfer.
Annual grants totalling up to $10,000 may be awarded to eligible practices on a competitive basis. Applications are independently reviewed by the Selection Committee consisting of general practitioners and physicians, with grants awarded to the project(s) deemed to best fulfil the aims of the Grant Program. Projects funded through the Grant Program must be completed within six months of receipt of the grant.
