CHGPN Grant Program

The Central Highlands, Victoria, General Practice Grant Program awarded annual grants between 2014 and 2017. The program was established through the transfer of retained equity from the Central Highlands General Practice Network (CHGPN) to Health Education Australia Limited (HEAL). The aim of the Grant Program was to support general practices in the historical CHGPN catchment to improve chronic disease management through systems development, capacity building, whole of practice learning and knowledge transfer.

Annual grants totalling up to $10,000 were awarded to eligible practices on a competitive basis. Applications were independently reviewed by the Selection Committee consisting of general practitioners and physicians, with grants awarded to the projects deemed to best fulfil the aims of the Grant Program. Projects funded through the Grant Program were completed within six months of receipt of the grant.
